AGE 16 OR OLDER
- Patients with multiple co-morbidities or chronic illnesses
- Younger patients with issues listed in Geriatric Medicine
- Patients with new onset dementia syndromes (<65 years of age)
- Undifferentiated symptoms and medical conditions
- Chronic cardiac failure
- Constipation/faecal impaction
- Chronic obstructive pulmonary disease (COPD)
- Drug and alcohol outpatient service
- Deep vein thrombosis (DVT) /Pulmonary embolism (PE)
- Dyspnoea on exertion
- Gout and pseudogout
- High INR (on warfarin) without bleeding or bleeding risk
- Electrolyte imbalance, including hyponatraemia (Sodium more than 130)
- Iron deficiency anaemia (if elderly patient or previously investigated) and Ferinject iron infusion service
- Osteoporosis
- Palliative care outpatient service
- Viral illness
